Dear Patient,
I would like to take this opportunity to thank you for choosing this office for your dental needs, and to tell you a bit about myself and the people who work here.
I have been practicing dentistry for about fourteen years. In 1984, I completed my B. S. in Biology as a member of the Psi Omega Honor Society at the University of Houston. I accepted a
position at Hermann Hospital Life Flight where I coordinated all of the ICU transfers from hospital to hospital. In 1992, I received my Doctorate of Dental Surgery from University of Texas
Dental Branch. Current, I am a member of the American Dental Association, Texas Dental Association, Greater Houston Dental Society and American Academy of General Dentistry. Personally,
I have been married for almost thirteen years and am the mother of two wonderful daughters: Leah and Eliza. I am also fortunate in my husband Lee, who uses his skills as an engineer to make
things work in a house full of women.
Every member of this office is dedicated to making your experience here a pleasant one by instituting a commitment to understanding. We appreciate how important it is to be able to make
an educated decision about your dental needs and we encourage questions. We also believe in keeping up to date with new dental technology so that the services you receive are of the highest
quality available.
Again, thank you for this opportunity to take care of your dental needs. We appreciate it.
Sincerely,
Ivis A. Hernandez, D.D.S., P.L.L.C.
